I'd add Qui-Gon and Dooku- he was a Jedi at one point and he set a lot of events in motion.

Luke and Anakin are automatic I think. The transition from Old to New Republic included the near annihilation of the whole Jedi Order and Luke and Anakin are at the center of that. Luke especially since he has to revive the Jedi after all that.
Yoda and Obi-Wan would probably be on the list since they were so important in Anakin/Vader's rise and fall. Yoda especially since he was so pivotal to the Order for so long.
However, if the Jedi Order is supposed to be 25,000 years old, I'd think that there would be other Jedi just as important at pivotal points in that history. Of course, we don't know who they are, since we don't have the whole story, but that's just a detail.

Yoda Anakin Skywalker Luke Skywalker Obi-Wan Kenobi Qui-Gon Jinn Plo Koon Revan (fallen Jedi from the Mandalorian & Sith Wars that came back to the light) Bastila Shan (hero of the Sith Wars) Odan-Urr (founder of original Jedi library) Vodo-Siosk Baas (original keeper of the powerful Tedryn holocron)

I don't comletely agree with the list: top 4 Yoda Luke since he brought back the Jedi Qui Gon Anakin

I agree, Qui Gonn should be in the list, he found the way for the Jedi to come back after life and be able to communicate with the living and then trained Obi Wan who then helped Luke.
