In The Viusal Dictionary to AOTC I believe it was, they came up with the 7 different forms of lightsaber fighting (this was a creation of the book, not of Nick Gilliard who admitted he had never geard of such a thing in one interview). And then there's Mace who came up with his own stle called "Vaapad" which teeters on the Dark Side. But then again that's Mace. Let me dig out my Visual Dictionary and see if I can get you a real run-down and not from my faulty memory

Form I: Very bacic. Teaches the user how to weild a lightsaber. The younglings in AOTC are being instructed in form I.
Form II: Developed specifically for saber to saber combat. Considered the best saber to saber combat form. That'd be Dooku. Very graceful and fluid.
Form III: Emphasises defense. Developed when blasters became popular and is very adept at deflecting laser blasts. Obi-Wan uses form III in AOTC and is still using it in ANH when he has his final duel with Vader.
Form IV: Very acrobatic and fast paced. Emphasises the use of the force to add amazing physical feats to extraordinary fighting skill. The flashiest form. Qui-Gonn and Yoda were form IV masters. Obi-Wan used form IV in TPM but switched to form III after Qui-Gonn's death because he felt the defense was inadequate and led to his master's death...
Form V: Focuses on the use of power and brute strength to overwhelm one's opponent. Anakin uses form V and still uses it as Darth Vader.
Form VI: Referred to as the "diplomats form". Similar to form III in that it is focused on defense, but form VI isn't aggressive at all. More focused on wearing the opponent down and striking only when all other options are exhausted. All the jedi who use form VI on Geonosis are killed.
Form VII: Form VII is very direct with no wasted motion. Strikes are aimed to kill or maim without any unneccessary movement. It also teaches use of the force to overwhelm your opponent using almost whatever advantage you can find. Form VII is difficult to master because it opens you so fully to the force its easy to let your emotions take control. This can lead to anger and aggresion which lead to the dark side. Only those with great control attempt to master form VII. Mace Windu is a form VII master.
They were also listed in an older issue of The Insider (fall 2002)

There are many ways to learn the ways of the Force, but only those who have joined it may know best.

The Saber combat styles also are in the "Powers of the Jedi" sourcebook for SWRPG.

The SWRPG "Heroes Guide" also has some bits on the Lightsaber Forms.
