It would certainly look incredible, but it'd take a combination of good tattoo artist, and True Believer in tattoos to get it done, as many artists won't tattoo on people's hands or even wrists unless they have pretty extensive coverage elsewhere, purely because of the social reactions to them... I recently saw a toy of Darth Maul with a training droid, which has him bare to the waist, showing how his facial tattoos actually extend over his entire torso and arms, and that's inspired me to get the tattoo across my shoulders extended down my spine as well ;D

Actually, if you look in the Episode III visual dictionary, there's a fantastic picture of Anakin's arm, although it is admitedly only the upper surface. That said, it shouldn't be too hard to extrapolate how the other surface would look from the pictures
