I found the Yoda black and white here:

there are other stickers there including the Jedi logos/Insignia. One is for the Jedi starfighters the other is for the Jedi them selfs. (or so it says)
The only other place ive seen that emblem was on a promo poster -

I have seen some of each of those stickers and pins on the shop Star Wars site.
Where the logo originally came from, I have NO idea.

Oh, i just remembered, it was on the licenced fabric that came out for Ep 3 as well.
