They definitely should've explained his cough, although from what I've read, I don't think Grievous actually had lungs... If he had, then vacuum would have been as damaging to him as it would a regular humanoid  Excerpt from a post by Andle Di-Ranos over at the Grievous Discussions thread: “His internal organs were enclosed in a layer of pressurized synthflesh with an organic fluid to prevent the organs from being damaged by bacteria and harmful germs, and also to maintain a suitable temperature to keep his organs alive and functional. His organs were nourished by artificial arteries keeping them alive with blood, allowing him to survive in a vacuum (in space, for example), an advantage he would display in escaping from Obi-Wan Kenobi and Anakin Skywalker on his flagship.”
