From what I've read in the novelization of ROTS, it said that the flesh and bone were melting, so I'm guessing the deformity was directly because of the force lightning being concentrated in just that area. I think the reference to the 'face of Palpatine' was more a reference to the psychological mask of acting like a nice guy, rather than just being Evil  Let's not forget, that in ROTS, Palpatine isn't a young man by any stretch of the imagination, so when he appears in ROTJ, there's another 20 years adding onto the deformity from the Force Lightning.
