They should make bracelets/chokers that are little cold or hot packs...
You could sew a fake sleeve that has a small pouch to hold the pack in place on your wrist.... or just shove it up your sleeve if it's tight enough.
Sharper image has that thing that you put around your neck that is supposed to keep you cool, but it looks like a giant electronic neckbrace.
